Position: Healthcare Social Workers
Location: 614 North Easton Road, Glenside, PA 19038
Working Condition: Full time / 40 hours per week
Job Duties: The Healthcare Social Worker position is responsible for the following essential job duties:

  • Assisting consumers to obtain medical / vocational services and opportunities for community involvement
  • Providing consumers with independent living skills instruction. Supporting and encouraging consumer participation in education, vocational supports, and/or competitive employment
  • Developing and enhancing consumer networks of social supports in the deaf and hard of hearing communities, with their families, in the neighborhoods, schools, and work places
  • Developing and implementing a discharge and aftercare plan for consumers in collaboration with their family, psychiatrist, treatment team and other relevant outside resources
  • Preparing assessments, new admissions, service documentation, termination summaries, incident reports, and miscellaneous paperwork/reports
  • Responding and handling crisis situations

 
Qualifications:
  • Education: Master’s degree in Counseling or in a related human services field
  • Experience: 12 months experience in the mental health field
  • Skills: Must be fluent in American Sign Language and possess an understanding of deaf culture and issues relevant to persons with hearing loss
